• Source identifiers and material claim mappings are retained with the article.
• Independent accuracy and clinical-safety audits must pass before human review begins.
• A named evidence reviewer, senior editor, and managing editor act through revision-bound workflow gates.
• Any edit invalidates earlier automated and human approvals.
• The AI pipeline has no publication permission.
